The flyweights Demetrious Johnson and Ian McCall fought, at UFC on FX 2, a very balanced fight full of emotion. But outside the octagon, members of the New South Wales Athletic Commission, responsible for sanctioning the event, committed one of the biggest gaffes in the history of Ultimate.

After 15 minutes of duel, Bruce Buffer announced the victory of Demetrious Johnson by majority decision, despite the notes read pointing to a split decision (29×28, 28×29, 29×28). Moments later, the event summary was presented and Anthony Dimitriou, one of the judges, had scored a draw at 29×29 and not McCall's victory. However, the turnaround in the result was announced at the show's press conference.

Judge Sal D'Amato, who had initially scored 29×28 in favor of Johnson, made a mistake when filling out the forms and his official score was 28×28, resulting in a draw.

As the UFC had already announced, in the event of a draw in the flyweight GP fights, an extra round would be held. However, with the incident, the organization announced that it will hold a rematch between the athletes and the winner will face off against Joseph Benavidez for the division's unprecedented belt.
